Transition Team

The Transition Team is based in the City Council's Children and Young People's Directorate, part of Children with Disabilities Social Work Service. The team offers a transition planning service for children and young people leaving full-time education. In partnership with other agencies, aims to support school leavers with social, educational and employment opportunities, 'bridging the gap’ between Children’s and Adult Services.
