A4e Support Planning service will help you to develop your own Support Plan for your local authority. Their Planners work closely with you to understand more about your support needs and the ideas you have to meet them and to look at creative ways to use the funding you have available to meet your needs.

Caters for all disability groups (physical impairments and/or sensory impairments, learning disabilities, users of mental health services, parents of children and young people, older people and carers).

Care2Care’s Support Planning service is available for older people, people with learning disabilities, people with physical impairments and/or sensory impairments, people with mental health issues and people with dementia who would like help to complete their Direct Payments Support Plans.

The Support Planning service assists individuals to identify what is not working in their life, how they want their lives to be and what changes need to be made to achieve that. The service will liaise and negotiate with service providers to ensure the Support Plans reflect their wishes and meet their individual needs.

The service includes the submission of the plans to Sheffield City Council for approval, and appeals where necessary, and the subsequent implementation of the selected services.

Communication Independence Support Planning prices range from £175 - £210. These prices are charged for the whole of the Support Planning service, including completing and revising a Support Plan and SPSOR.   Community Action Halfway Home - Support Planning and Brokerage
Community Action Halfway Home is a registered charity that has been providing mental health services in Sheffield for over 35 years. Provides practical and emotional support to people who are experiencing mental health problems, to enable them to establish an independent life. A support planning service is available for clients who have been assessed for Self-Directed Support (SDS) or Direct Payments. A client centered, holistic approach is taken, enabling clients to think about what they want for their future. The plan sets out what clients want to achieve, how they intend to reach their goals and what kind of support they would find most helpful.

The services identified in the plan are discussed with the client, priced and then forwarded to the care coordinator for submission to the panel, who can then agree that it meets the client’s assessed needs. Once the support plan has been agreed, the personal budget can be released.

Prices:

The Support Planning service can be purchased using a SDS budget. There is a standard charge of £125, this covers up to 2 meetings with the client, pricing services and writing the support plan for the care coordinator to sign off and submit to panel. Any additional work will be charged at £27 per hour plus expenses.

Halfway provides a Brokerage service to make the Support Plan happen. Once the Support Plan has been written and a budget has been agreed, experienced staff will help the client to contact services to access the services identified in the Support Plan and help with all processes involved in accessing the services. This may include initial contact with the service provider, help completing referral/application forms, acompanying clients to interview and ensuring client attends first appointment with service provider. There is a standard charge of £160.

The Support Planning service can be purchased together with the brokerage service using the SDS budget. The cost is dependent on the number of services identified in the support plan.

I-decide is a social enterprise that provides personalised, community-inclusive support solutions for individuals with a support need including older people, people with physical and/or sensory impairment, people with learning disabilities and people with mental health issues.

As a wholly owned subsidiary of the charity Deafblind UK, which has been at the forefront of supporting people with a sight and hearing loss for more than 80 years, they offer specialist knowledge and an understanding of sensory impairments as well as disabilities across the spectrum. Launched in 2012, I-decide is part-funded by the Department of Health’s Innovation, Excellence and Strategic development fund 2012.

I-decide believes that truly personalised services, shaped and tailored to meet the needs, wishes and aspirations of the individual, can only occur when that person is at the centre of planning and designing their own support plan. It is essential to have lifestyle choices and aspirations and i-decide helps individuals to retain control and ownership of their support package. An expert Support Planning and Brokerage team provides advice and support at every stage of the process using all types of communication methods to meet the individual’s needs. Aims to educate customers to ensure the sustainability of Direct Payments and Individual Budgets for the long-term..

I-decide will work with you to appoint exactly the right people and turn your support plan into a reality. Supporting you in employing a PA, I-decide will help you:

I-decide can collect your Direct Payment from your Local Authority and any assessed contribution of ILF funding and/or your own contribution. Can manage your funds and make payments to your Personal Assistant, care agency, HMRC and any other Direct Payment related invoices.

I-decide also provide you with real time access to your account online and provide income and expenditure statements and reconciliations on your behalf to HMRC.

• helping to draft letters and other documents as and when needed

IDK Support 4 You is a service for people (including older people, people with learning disabilities, physical impairments and/or sensory impairments, people with mental health issues and people in transition) in Sheffield who have received an Indicative Personal Budget following a needs assessment for Self Directed Support and who are looking for help to complete their Support Plans, manage their budget or need support with payrolls. Support brokerage is a wide term, covering a set of functions which can be fulfilled in order to support an individual to reach the outcomes which they have set for themselves – therefore Self-Directed Support. For example, for an individual who is in receipt of a Personal Budget, Imagineer can contribute by assisting the individual in realising their dreams, and getting these dreams written into their Support Plan, to ensure that their Personal Budget can enable these dreams to become a reality.

• Brokerage Management
This is the service which follows on from an individual receiving a Support Plan and having their Personal Budget established, and has several aspects such as providing on-going management, such as establishing and organising a payroll system.

• Beat It
This project provides sessions in all aspects of music, dance and drama for adults and young people with learning disabilities, learning difficulties and poor mental health. These sessions allow people to be creative in a positive environment which allows them to build their confidence, learn new skills and meet new people. This can be purchased via their Personal Budgets, if such activities form a part of their Support Plan, or can be paid for out of their own resources.

Through Your Choices is similar to Beat It, however provides music, dance and drama sessions at an introductory level, in attempt to pique interest, which may lead to an individual focussing upon this aspiration at the Beat It sessions. Through Your Choices (T.Y.C) also provides one-to-one support and group activities within an individual’s Circle of Support, to help these individuals attain their dreams and aspirations which they may have previously felt were unattainable. This is achieved by listening to the individual, and what they would like to achieve, and formulating a strategy to achieve these goals together, step by step. By tackling dreams and aspirations in small chunks, the confidence of both the individual requiring support and their Circle of Support can be achieved, which in turn increases the likelihood of repeat occurrences of successful outcomes.

This price includes:
• Time spent with you to understand what your needs are and how you would like them to be met
• Time it takes to look at what is available to meet your needs
• Time it takes to write your Support Plan in an accessible way

Leonard Cheshire Disability Sheffield Support at Home offers a Support Planning service for individuals in receipt of Direct Payments or an Individual Budget.

Sheffield Support at Home Support Planning Service is for anyone with a support need including older people, people with physical impairments and/or sensory impairments, learning disabilities and people with mental health issues.

To enable the service to devise a Support Plan catered to your support needs, Support at Home works in a person-centred way that places your needs and aspirations at the centre and makes sure you are involved in the development of your Support Plan at every stage.

Services Provided:

The Support Planning service includes:
• mapping out your individual needs and identifying who can meet those needs
• developing a Support Plan for submission
• support to arrange and meet service providers
• support to attend service provider meetings

Quality assurance reviews are carried out after 6 weeks after the support plan has been agreed.

Money Management Service:

Penderels Trust is an established provider of Direct Payments and Personal Budget advice with over 20 years experience of working with people with disabilities to enable them to live independently and be active in their local community. The Trust is a not-for-profit organisation.

The Trust provides support services locally in Sheffield and supports all disability groups including: physical impairments and/or sensory impairments, learning disabilities, disabled parents in a parenting role, older people, mental health service users, people living with HIV/Aids related illness, children and young people, carers, young carers and families.

The Rowan Organisation is a charity that has over 30 years experience of working with people who are in receipt of Direct Payments and Self Directed Support including people with physical impairments and/or sensory impairments, learning disabilities, mental health issues, older
people and children. The charity is a User Led Organisation (ULO) with
over 75% of its Board of Management being service users. The Rowan
Organisation has Sheffield City Council's Recognised Provider List status for all it's services except the support planning service.

The Rowan Organisation’s philosophy is to provide support to enable customers to manage their Personal Budgets and/or Direct Payments packages independently. However Rowan recognises that for various reasons this may be difficult for some people and therefore provides a Managed Account Service. The Rowan Organisation administers income
and expenditure whilst the customer remains in charge of the services

The Rowan Organisation can carry out an enhanced DBS disclosure on PAs on behalf of the employer.

• Sending out paperwork for the employer to verify the documentation sent in to support the DBS application
• Returning all verification documents directly to the applicant
• Countersigning the DBS application and forwarding to theDBS

The Rowan Organisation payroll service can provide weekly, fortnightly, four weekly or monthly support. To reduce the cost of the service four weekly or calendar monthly pay periods are recommended.

• HM Revenue & Customs (HMRC) registration as an employer
• Deal with all correspondence from HMRC including end of year on-line submissions
• Calculate all payments to employees as required
• Provide payslips
• Calculate HMRC payments
• Retain annual leave records for employees

If you wish to recruit PA’s independently without support from The Rowan Organisations local team, you can use the Recruitment Line; this is an online-based service.
The Recruitment Line will:
• Send an advert information form for you to complete and return
• Send information sheets on how to write job adverts, job descriptions and person specifications
• Place the advert on the Rowan Organisations website

  Stocksbridge Brain Injury Rehabilitation Centre
A rehabilitation unit which caters for people with physical disabilities, brain injuries, epilepsy, challenging behaviour and strokes. Also caters for young adults.

The centre has an occupational therapy team to assist with social, cultural and learning opportunites. There are gardening projects on site for patients to take part in. Transport is available for access to local services which is included in the weekly fee.

As part of moving on from the unit the centre can provide:

Options offers a complete personalisation service, from initial budget and support planning, right through to receiving the actual support. Options aims to support their customers, based on what clients say is important to them. Support Plans assess risk and ways to minimise it.

SYHA Options provides a Brokerage Service, finding out how much money you are entitled to, planning the best ways to spend it, getting everything agreed with the local authority, organising money and support.

Options will work with both customers and their representatives to understand each individuals needs:
• assess how much money they are entitled to
• choose to spend the money in ways and at times that are tailored to an individuals needs
• agree outcomes that must be achieved through receiving the right support.

SYHA Options can support customers to employ a personal assistant.

SYHA Options provides a Support Planning and Brokerage service for adults with learning disabilities, people with physical and sensory impairment, people with enduring mental health issues and older people.
