  Care2Care - Dementia Day Care


Care2Care Dementia Day Care is a day care service for people with dementia (including older people with dementia and early onset dementia), including those in receipt of Individual Budgets.

Services Provided:

The day service offers a friendly place for people with dementia to socialise and take part in activities. A variety of hot and cold lunch choices are on offer, with refreshments served throughout the day. The service runs a planned programme of social activities to provide entertainment, stimulation and aid rehabilitation. Door to door transport is provided for all clients.

Sessions are held at Roman Ridge, Lavender Way, S5 (Monday, Thursday) & Sorby House, Spital Hill, S4 (Wednesday).

Prices:

£60 per day, includes meal, refreshment and transport (prices are provided by Care2Care Services, please check them yourself for accuracy).

  Cinnamon Trust, The
Cinnamon Trust is a registered national charity which helps elderly and/or terminally ill people look after their pets. The Trust has a network of 12,000 volunteers for dog walking for housebound people, fostering pets when their owners are in hospital, fetching cat and dog food and cleaning out a bird cage etc.

They keep a list of pet-friendly care homes and take on the care of pets whose owners have passed away, providing prior agreement has been made by completing Pet Profile cards. The Trust provides help with any pets and also provides vet transport . Application forms can be downloaded from the website. Volunteers from the Sheffield area are welcome.

  Dore and Totley Day Centre and Lunch Club
Lunch club and day centre for older people and housebound people. Organises annual summer and Christmas meals out. Cost is £4 per session. Transport is provided. Lunch club held at Dore and Totley United Reformed Church, S17 (Tuesday).

How do I get there?
Car parking is limited. There are limited places on the clubs own transport for people with walking difficulties. Bus numbers 97 and 98 run between Ecclesfield and Totley/Totley Brook via the city centre and run along Abbeydale Road.



  Dore Open Door Lunch Club
A small friendly lunch club for older people, housebound residents of Dore, manned by volunteers. Caters for 12-15 elderly members with transport provided by Transport 17 minibus. Organises outings for lunch and a Christmas lunch for all members and volunteers. Waiting list in operation. Lunch club held at Dore Old School, S17 (Tuesday).

£3.50 and £1.50 for transport (please check the price for accuracy).

Car parking available. There are places on the clubs own transport for people with walking difficulties. Bus number 70 runs between the city centre and Dore, the nearest stop is Devonshire Terrace Road Terminus.



  Future Years Group
Part of Sheffield City Council. A day centre for older people with learning disabilities providing relaxed, quiet activities, games, baking and other projects. Transport can be arranged. Has 15 places, however there is a waiting list in operation.


  Gleadless Valley Methodist Church Luncheon Club
The lunch club provides a home cooked meal and activities for housebound older people. Transport is provided. There is a waiting list in operation. Lunch club held at Blackstock Road, S14 (Monday).

  Healthcall Optical Services
A local branch of a national private company providing home sight tests and aftercare. The Optical Service is free to those eligible for free NHS eye tests. Provides: home delivery and fitting of spectacles for elderly or people with physical disabilities; vision screening and eye examinations for company employees; domiciliary optical services to the housebound in their own homes or to care homes.


  Helping Angel, The
Karen McNamara is a sole trader providing Personal Assistant (PA) services to the housebound, older people and people with a physical disabilities and/or sensory impairments.

Services Provided:
Karen offers the following 3 services:
• A Shopping and Delivery Service - Karen can help you to make a shopping list, purchase the items on your behalf and put away all the shopping upon her return
• A Sitting Service - Karen can sit with a client who requires company or who can not be left alone, allowing their carer to get out and about for a while
• Escorted Trips - Karen can escort clients getting out and about on trips, outings, going shopping, social activities and hospital visits etc.

  Home Calls
Home calls provides a daily telephone service to older people living independently. The service calls at an agreed time everyday to check that the client is safe and well and that there are no problems, issues or concerns within the home. If there are any issues an alert goes out to family, neighbours, doctors or emergency services as required.

A regular short chat and a laugh helps to relieve loneliness and isolation, and may save lives. Home Calls aims to build a relationship between carer and service user. The daily telephone call gives peace of mind to the service user, family members and carers. The service can also remind clients about medication, appointments etc.

There are 3 levels of service, gold, silver and bronze. Monthly charges start at £30 for bronze level (service provided Mon-Fri only), £47 for silver level (service provided 365 days a year excluding bank holidays) and £50 for gold level (service provided 365 days a year including bank holidays), (these prices are provided by Home Calls please check them yourself for accuracy).

  Home Library Service
The Home Library Service delivers all library materials to anyone who is unable to use the traditional library service for any reason. A limited video/DVD service is provided free of charge.

  Mobile and Home Library Service
Part of Sheffield Libraries, Archives and Information. Special function is to facilitate access to library and information services for those people unable to use a building based service. All services are outreach, working with communities and with individuals.

Also includes materials for adults, with a large selection of books in large print and on tape, jigsaws, videos, music casettes, DVDs and CD's. Monthly delivery and collection door to door.

The Mobile Library Service stops at 98 locations around the city. Details can be found on the website or telephone for details.



  Muslim Elders Day Service

The Day Care service for Muslim Elders was established following requests from the Muslim community and is delivered by Care2Care Services in partnership with Sheffield City Council.

Services Provided:

The aim of the service is to provide:
• a day care service for Muslim Elders, older people age 65 years and over, which caters for all the city's Muslim communities
• a safe, supportive and structured environment, where individual's needs can be met and they are encouraged to develop independent living skills
• a range of personal care services and appropriate activities to improve and sustain a quality of life that promotes dignity and encourages service users to achieve their maximum potential
• a culturally sensitive service that will respect and celebrate the cultural, ethnic and religious beliefs of service users and continue to develop in response to their wishes
• a culturally sensitive service that gives full consideration to gender segregation, language differences and the differences between the varied ethnic groups in the Muslim community

The service provides refreshments and a varied lunch menu, with a range of halal and vegetarian options.

Transport to and from The Meadows day care centre is provided. Pick up times for this service start at 9am.

Day centre facilities and services include:
• Laundry - a laundry service is available on site at The Meadows. Service users are able to use these facilities as required. Care staff will ensure that any washing brought by the service user is washed, dried and ironed by home time. A fee will be charged to cover the cost of providing this service.

• Bathing - bathing facilities will be provided for individuals wishing to have a bath whilst at the centre. Trained staff have access to a private, fully accessible specialist bathroom and will provide bathing assistance if required. Respect for individuals' gender, culture and beliefs will be given at all times, as will the maintenance of privacy and dignity. A fee will be charged to cover the cost of providing this service.

• Activities - all activities will be varied and individualised to suit service users' preferences. Care staff will provide service users and their carers with the opportunity to find out about the variety of social activities the service has to offer and a weekly activity programme will be incorporated into individual care plans. Regular trips out will be offered to provide the opportunity to visit different places and events of interest. A fee will be charged to cover the cost of trips.

• Personal Care - information will be provided about hairdressers, dentists, opticians, chiropodists and other personal care services. Care staff will be happy to arrange visits for anyone wishing to use these services whilst at the centre.

• Health Care - staff will work with service users and their carers to ensure the person has full medication support to ensure it is taken correctly and safely at the appropriate times.

• Spiritual Care - information regarding the facilities available for worship will be made available to all service users. Care staff will endeavour to assist and lead worship or prayer times as required.

Qualifications & Quality Standards:

Care2Care is registered with and inspected by the Care Quality Commission (CQC) to ensure a quality service is provided. All staff have an Enhanced CRB check prior to commencing service delivery.

Sessions are held at The Meadows, 30 Busk Meadows, S5 (Tuesday).



  Oakhouse Foods
Oakhouse Foods is a company providing a frozen food home delivery service to the elderly throughout Sheffield. They offer a range of more than 300 frozen products from ready meals to desserts. Oakhouse can cater for food allergies or intolerances and other preferences. In addition they can deliver over 500 grocery items including fresh milk, cheese and butter.

To place an order, simply choose the meals and desserts on the website or telephone. Locally-based drivers will bring your order to your door on a regular delivery day and normally at around the same time of day. The driver will also be happy to put your order straight into your freezer if required.

Prices vary from approximately £2.30-£4.99 for a main meal and £1.40-£4.25 for a dessert (please check website for individually priced meals). There may be a small delivery charge of £1 for orders under £30.



  Park Care Meals
Park Care Meals provides hot meals at lunchtime and can provided a cold meal for tea-time.

They can cater for special dietary needs such as low salt, diabetic, gluten free, vegetarian and soft texture foods.

Costs are £5.40 for a hot dinner and a dessert. £2 for a cold meal for tea-time. These prices have been provided by Park Care Meals, please check them yourself for accuracy.



  Podiatry Service
Part of NHS Sheffield. Provides general and specialist podiatry treatment in Sheffield, based on clinical/medical need. Services include nail surgery, podiatric surgery, community and hospital based diabetes care, rheumatology clinics and special needs clinics.

All referrals are triaged by a senior podiatrist and offered an assessment appointment according to risk status. High risk patients are seen within 2 weeks of receipt of referral and low risk patients within 12 weeks. Also takes self referrals.

Services are provided in community clinics, some GP clinics, hospitals, a mobile clinic and at home for housebound patients.



  SACMHA

SACMHA is a health and social care company that provides a range of support services for older people, people with learning disabilities, people with physical impairments and/or sensory impairments, and people with mental health issues including those who are in receipt of Direct Payments or an Individual Budget. All services can be tailored to meet the needs of the individual.

The company has many years experience in catering for older people, people with mental health issues and providing a culturally sensitive service to clients with BME backgrounds. See also separate entry SACMHA - Support Planning and Brokerage.

Services Provided:

SACMHA provides help with every day life and chores providing a one to one practical support package to help with everyday practical tasks, this does not include personal care.

Domiciliary Support:

• going shopping for the day
• meeting up with friends and family or attending appointments

• holiday support, going on a short break
• playing board games
• using public transport
• going to church

• SACMHA provides Community Outreach - a flexible service to help with managing your mental health, admin (correspondence and letters), bills, tenancy support, appointments etc. This can be in your home or in the community.

• Carers Support - one to one support to the socially isolated, this includes: group activities, therapeutic groups, support with finance and budgeting, support to access other services and services for those you care for.

• Befriending and sitting service.

• Advocacy Service (sees separate entry) - for people experiencing mental ill health, the service will not only advocate on your behalf but will help you to develop the confidence and skills to manage situations that may arise in the future.

• Getting the Best From Today's Technology - a service specially designed for those who would like to purchase one to one technology support to learn: how to use computers, access the internet, set up a DVD recorder or digital TV, use a mobile phone or choose the right computer equipment.

• Lunch Club and Drop-in Centre
SACMHA also runs a lunch club and drop-in centre (see separate entry), a carers support service (see separate entry), supported accommodation (not available through Self Directed Support), service user groups and a range of social and educational activities.

• Meals Where You Are
SACMHA provides meals which can be picked up or delivered for a small extra charge. Meat is halal and meals can cater for all dietary needs. Meals can be pre-ordered in single portions and can be frozen. A full menu of meals is available.

• Supported Accommodation - providing vulnerable people with accommodation in one of SACMHA's 10 supported living flats in various locations around the city pre or post hospital admission. This service is delivered following a detailed assessment, jointly conducted with colleagues in the Statuory Sector.

• Community Resource Centre - the Centre is also SACMHA's administrative base and provides access to IT, tasty Caribbean meals, a games room and a large garden. People from all kinds of backgrounds can use this Centre as a "One Stop Shop" for a range of health and social care information. The venue also has rooms available to book for meetings and functions.

• Community Outreach costs £12-17 per hour depending on the level of support needed (an additional charge may be made for unsocial hours and weekends)

  Sheffield Salaried Primary Dental Care Service
Provides dental treatment for patients who have difficulty in obtaining treatment elsewhere. The service works in conjunction with local family dentists. SPDCS provides a service for people of all ages with special needs and elderly people.

Treats housebound people without a family dentist if they are referred by a Health or Social Care Worker. Adults and children who have a dental phobia and who need a special introduction to dentistry to prepare them for treatment by a family dentist may access the service if they are referred by a health or social care worker. Dental health education may be provided for priority groups, held in locations throughout the city.

Clinics are located in S5, S6, S8 & S12.



  Special Needs Housing and Care Agency (SNHCA)
SNHCA provides a culturally sensitive service offering home care and support to minority ethnic community members, including older people, people with physical impairments and/or sensory impairments, learning disabilities, people with mental health issues and housebound in Sheffield.

Services Provided:
Services include help with housework, cleaning and shopping. Can help clients to find suitable accommodation for their needs and provides an interpreting service to help deal with authorities.

Prices:
The service charges £17.50 per hour, there is no extra charge for public and bank holidays (this price is provided by SNHCA, please check it yourself for accuracy).

Areas Served: The service is citywide covering S1, S2, S3, S4, S5, S6, S7, S8, S9, S10, S11, S12, S13, S14, S17, S20, S35 and S36.

  St Chads Church 3rd Age Ministry - Befriending
A voluntary befriending scheme where church members visit housebound or lonely older people, aged 50 years and over, once a month. The scheme is confined to people living in Woodseats, Beauchief and Greenhill.


  St Timothys Lunch Club
A warm and friendly lunch club with home cooked food. Catering for older people aged 60 years and over. Transport can be provided for people who are housebound or have dementia who live within a 1 mile radius of the church in Crookes. Referral by health professional or Social Services.

Lunch club held at St Timothys Church Hall, S10 (Wednesday).

  The Armchair Spectacle Company
The Armchair Spectacle Company are a locally based opticians who provide eye tests for people in their own home in the Sheffield, Barnsley and Rotherham area.

The service offers home eye tests to patients who are housebound or who have difficulty getting to their normal opticians unaided, as well as private tests for those who would simply rather be tested at home.

Prices:

Eye tests are free if the patient is eligible under NHS criteria otherwise there is a charge of £30 (this price is provided by The Armchair Spectacle Company, please check it yourself for accuracy).

Qualifications & Quality Standards:

All staff are registered with and checked by the local Primary Care Trust (NHS body). The opticians are fully qualified, CRB and background checked and have years of experience carrying out eye tests in patients own homes. The service uses the latest portable equipment to ensure a thorough test just like in the optician on the high street but in the comfort of your own home.



  Wiltshire Farm Foods
Wiltshire Farm Foods is a company that prepares and delivers frozen ready meals to people. There is a choice of over 300 nutritious main meals and desserts. Special diets can be catered for including low fat, moderate salt, diabetic, gluten free, vegetarian, ethnic & texture modified diets. Customers can order meals on a regular or occasional basis.

The Wiltshire Farm Foods drivers wear a company uniform, carry an official identity card and have undergone enhanced checking with the Criminal Records Bureau (CRB). Meals can be unpacked and put in the freezer for you if required.

The majority of the meals and desserts can be heated in the microwave in minutes – ideal for time-conscious carers who need to provide a meal.

Prices vary between approximately £2.50-£4.99 for a main meal and £0.85-£2.50 for a dessert (please check website for individually priced meals).



  WRVS - Meals Delivery Service
WRVS offers a delivery service of frozen meals for lunch, as well as sandwiches etc for tea time. Staff can help with changing light bulbs, posting letters, putting food in the freezer and contacting relatives if a client is not feeling very well. Can ceter for special dietry needs
